Hydrogen peroxide reacts with melanin in hair, removing the color in an irreversible chemical reaction. The hydrogen peroxide oxidizes the melanin molecule. The melanin is still present, but the oxidized molecule is colorless.  Although the melanin is colorless, the hair tends to have a pale yellow tint due to the the natural color of keratin, the structural protein in hair.

Hydrogen peroxide is used to whiten teeth.  The white of teeth are protected by an enamel.  This is made up of hydroxyapatite crystals, which form microscopic hexagonal "rods." Simply put, enamel is porous, which means staining agents such as coffee, tea, cigarette can work their way down into the tooth, where you can't simply brush them away. The deeper stains are not harmful, but some people find them unpleasing.  Hydrogen peroxide is utilized to remove these stains.  Basically, hydrogen peroxide is used to get down into the tooth enamel and react through oxidation, which cleaves the staining compounds.  Peroxide is utilized in toothpastes, mouthwashes, tooth whitening gels and strips, and is used both professionally and in home use.
